The Vermont Department of Disabilities, Aging and Independent Living (DAIL) decides if you have physical issues that make it hard for you to do daily activities. Daily activities are things like getting in and out of bed, standing, walking, grooming, dressing, bathing or eating. A nurse from DAIL will visit you to decide whether you need help with these activities.
Your Choices for Care application is complete once both DAIL and ESD have all the information that has been asked for. Make sure that your application is complete. You can do this by checking in with DAIL at 802-241-2401 and ESD at 1-800-479-6151.

When a decision has been made on your application, you will be told in writing.
You have the right to appeal a decision that tells you that you can’t get long-term care services through Choices for Care.
