- Print the 201P application, or
- Call 1-800-250-8427. Ask to get a 201P application in the mail.
- You will be considered for both VPharm and Healthy Vermonters.
If you need to apply for prescription help and Medicaid or a Medicare Savings Program:
- Print the 205ALLMED application, or
- Call 1-800-250-8427. Ask to get a 205ALLMED application in the mail.
- You will be considered for both VPharm and Healthy Vermonters, as well as Medicaid or a Medicare Savings Program.
