You can apply for the Choices for Care Long-Term Care Medicaid program if you are 18 years old and over and need a “nursing home level of care.” A nursing home level of care means you need a lot of help or total help with activities of daily living. “Activities of daily living” include dressing and undressing, bathing, personal hygiene, bed mobility, toilet use, transferring, mobility in and around the home, and eating.

The Choices for Care program helps pay for care for older Vermonters and people with physical disabilities. It may be able to help you with some of the costs of your care.

Get help from the Council on Aging

You can get help applying for Choices for Care by contacting the Council on Aging. They provide support and services to Vermonters age 60 and older. There are five Councils on Aging in Vermont – including one for your area. The Council on Aging has a toll-free, statewide Senior Helpline at 1-800-642-5119. The Senior Helpline is confidential. You can call the Senior Helpline for help with applying for Choices for Care.
